Having her instinctually cover up her "bad" side with her hand now and then even though it's now fine is an impactful affect.

I understand (because of Topbilled) that the source material sets the story in Vienna, but it would have been interesting had they moved it to New York and given it a modern '40s feel versus the "old world" tone of the one.

You're correct, plus the dialogue and tone would change too as this movie feels more 19th than 20th century (other than the modern plastic surgery, of course).

There is no way the production code would have allowed her to kill the boy. But this is still a very suspenseful sequence.
Cukor is on par with Hitchcock here.
